Site Registration Form
Use this survey to submit confirmation of placement at a site that appears in the Undergraduate Practicum Guide. You must have confirmation from your prospective site supervisor, prior to submitting this survey.
To complete enrollment in Undergraduate Practicum (PubHlth 195) you must enroll in the lecture and discussion components of the course. All students must have a site registration form submitted prior to the deadline for the quarter of your planned enrollment.
- Please note a confirmation is a secured offer from a site. You will need to have completed your interview process and received a final offer of a confirmed internship prior to submission of your registration.
